Writing a Private Prescription

All doctors with a medical license are able to write a private prescription. 


They are allowed to write prescriptions for non-controlled substances for themselves, family, friends or patients. 


Obviously - clinical judgement and safe practice is essential - making sure to only prescribe medications which are indicated and that you are familiar with.


What you will need to write a prescription:


A STAMP

In hebrew: חוֹתָמֶת - chotémet (kho-TE-met)

You can buy a stamp set in most stationery shops or order a ready made stamp online.

What you must include on your stamp:

  • Full name (In Hebrew)

  • Medical license number 

    1. This is the number you’ll find on your paper medical license that you will have received from the Health Ministry (Misrad HaBriut)

    2. It starts with a 1 and is followed by 3-6 digits, like this: 1-123456  


A PIECE OF PAPER

This can be any piece of paper or a printed template. 


A PEN

This can be any colour, ideally black or blue. 


How to layout the prescription:

The information that should be included on an Israeli prescription is very similar to every other country.


The prescription can be written in English or Hebrew


The format is simple and should include the following details:


Header

Prescription 

מִרְשָׁם 

Mirshám - meer-sham

Date

Day / month / year

Patient’s full name


ID number

Passport number or governmental ID card

Drug name and formulation

Amoxicillin 500mg Tablets

Instructions for use

1 tablets x 3 /day x 7 days a

Quantity to provide

Provide 21 tablets
